How we help

The Pathways to good health service offers health screening, assessment, planning and referral support for children and young people entering or re-entering out-of-home care. We offer:

  • Health checks and planning for children and young people entering or re-entering kinship, foster or residential care
  • Support from health navigators who work alongside children, carers and support team create tailored care plans
  • Referrals to our specialist assessment clinic, including paediatricians, speech pathologists, occupational therapists and child psychologists.

Together this team delivers timely, collaborative, trauma-informed and child-centred care. The Pathway to Good Health service is developed and funded by the Victorian Department of Health, in collaboration with the Department of Families, Fairness and Housing and in the outer east is delivered through a partnership between Each, Eastern Health and Access Health & Community.
